From 1979 to 2013 board certified members of the American Board of Dermatology, the American Board of Otolaryngology, The American Board of Ophthalmology with completion of an American Society of Ophthalmic Plastic and Reconstructive Surgery, Inc. (ASOPRS) fellowship and the American Board of Oral and Maxillofacial Surgery who were interested in Cosmetic Surgery were able to challenge the American Board of Cosmetic Surgery annual examination in the area of Facial Cosmetic Surgery and Dermatologic Cosmetic Surgery. Over the course of the 34 years this pathway was available to hundreds of members from the above boards who undertook the additional training necessary to obtain the coveted status of Board Certification in Facial Cosmetic Surgery by the American Board of Cosmetic Surgery, the only board in North America devoted exclusively to Cosmetic Surgery.

In 2010, the American Board of Cosmetic Surgery, at their annual Strategic Planning Session, voted to examine and certify in a single area of certification which would include Facial Cosmetic Surgery, Dermatologic Cosmetic Surgery and Body, Breast and Extremity Cosmetic Surgery under the name General Cosmetic Surgery. The decision was made to implement this policy effective 2014. Accordingly, the pathway followed by many of the members of the American Board of Dermatology, the American Board of Otolaryngology, The American Board of Ophthalmology with completion of an American Society of Ophthalmic Plastic and Reconstructive Surgery, Inc. (ASOPRS) fellowship and the American Board of Oral and Maxillofacial Surgery came to an end in 2013.

The result was there no longer existed a surgical certifying board in North America that was devoted exclusively to offering certification in Facial Cosmetic Surgery. Interested candidates examined the possibility of certification by the American Board of Facial Plastic and Reconstructive Surgery (ABFPRS), however, the ABFPRS candidates were limited to members of the American Board of Otolaryngology and the American Board of Plastic Surgery. Members of the American Board of Dermatology, The American Board of Ophthalmology with completion of an American Society of Ophthalmic Plastic and Reconstructive Surgery, Inc. (ASOPRS) fellowship and the American Board of Oral and Maxillofacial Surgery did not have the opportunity to challenge the ABFPRS annual examination.

The concept of creation of a medical certifying board in Facial Cosmetic Surgery came about when representatives of the American Board of Dermatology, American Board of Otolaryngology, the American Board of Ophthalmology with completion of an American Society of Ophthalmic Plastic and Reconstructive Surgery, Inc. (ASOPRS) fellowship and the American Board of Oral and Maxillofacial Surgery met and recommended that an independent examining body be organized to develop a credentialing and examination process in Facial Cosmetic Surgery.

The American Board of Facial Cosmetic Surgery was incorporated on February 10, 2015 and recognized by the Internal Revenue Service as a IRC § 501(c)(6) non profit organization. The first separate certifying examination for the American Board of Facial Cosmetic Surgery was given in October, 2015 in conjunction with the annual certifying examination for the American Board of Cosmetic Surgery. Each year the ABCS and ABFCS jointly administer their examinations and separately certify their Diplomates.
